- Please fill out the new patient referral form below. The form will securely download to our office through a HIPAA-compliant system. If you’re unable to complete it online, reach out to us, and we can provide a printable version via email or fax.
- Include the most recent progress notes detailing current medications and any pertinent lab results. If uploading progress notes via our secure form isn’t possible, please fax them to our office.
- Once authorization is obtained, we will contact your patient to arrange their first appointment. Authorizations are typically processed within 48 hours
El Paso New Patients
Below are our general referral guidelines along with our online intake form for new patients.
Our primary patients have Medicare Part B coverage. While a medical referral is not required, we do ask that our new patient intake form is fully completed. Once we receive the intake form, we will verify insurance eligibility and complete any necessary prior authorizations.
How to refer a patient
Referral Guidelines:
- Patients with wounds that have not healed within 4-6 weeks.
- Patients with open wounds measuring 1 cm or larger.
- Wounds that have shown less than a 50% reduction in size over the past 30 days.
- Our primary patients have Medicare Part B insurance.
Coverage Information:
- We are not classified as home health and bill through Medicare Part B.
- Patients with traditional Medicare Parts A and B can receive both home health services and our specialized wound care simultaneously.
- A Medicare ID number begins with a number, followed by a letter, and is 11 characters in length (e.g., 1EG4-TE5-MK72).
- Please note that Medicare Advantage Plans do not cover our treatment plan
